Care Navigation

Initial Care Navigation Visit — $295

60 minutes

A comprehensive visit to understand your healthcare concerns, current care, goals, and the areas where you need additional guidance or support.

May include:

• Review and organization of healthcare information
• Help understanding diagnoses, recommendations, or treatment options
• Preparation for upcoming healthcare appointments
• Questions to discuss with your healthcare team
• Navigation of referrals, specialists, and next steps
• Personalized navigation plan

Individual Follow-Up Visit — $125

30 minutes

Flexible follow-up support when you need help navigating a new concern, preparing for an appointment, understanding next steps, or revisiting your care plan.

Care Navigation Membership — $325/month

Ongoing personalized support for individuals and families who would benefit from regular access to care navigation throughout the month.

Membership includes:

• Two 30-minute Care Navigation visits each month
• Brief secure messaging between visits
• One-business-day target response time
• Review of relevant healthcare records
• Appointment preparation and debriefing
• Ongoing maintenance of your personalized navigation plan
• Limited coordination with your healthcare team when authorized and appropriate

Additional Member Visit — $125

30 minutes

Members may schedule additional Care Navigation visits as needed at a reduced member rate.

Clinical Consulting

Customized Consulting for Healthcare Organizations

Clinical consulting services are tailored to the needs, scope, and goals of each organization. Following an initial consultation, VEYA will provide a customized proposal outlining the recommended scope of work, deliverables, timeline, and project fee.

Consulting services may include:

• Clinical policy and procedure development
• Policy review and revision
• Clinical workflow assessment and redesign
• Quality and process improvement
• Evidence review and translation into practice
• Implementation planning and support

Payment & Insurance

VEYA Health & Consulting offers transparent, self-pay pricing for Care Planning and Care Navigation services. Payment is due at the time of service unless otherwise arranged.

VEYA does not bill commercial insurance for these services. A superbill may be available upon request; however, reimbursement by your insurance plan is not guaranteed.

We understand that plans change. Please provide at least 24 hours’ notice when cancelling or rescheduling an appointment. Late cancellations and missed appointments may be subject to a fee.
